Why not without desks?

I am so transparent, aren't I. Imagine the librarian [doctor] roaming all over campus meeting the user [patient] where their need is; imagine roaming from government office to government office meeting the city officials need; imagine roving from park to park being ubiquitous ; imagine a set period time you spent out of your office finding out what people needed, rather than believing they will come to you --oh the places we could go!
